Diabetes/Lipids MDT

We facilitate the delivery of monthly diabetes/lipids MDT. This involves GPs and other clinicians bringing exceptional cases to the meeting to discuss/formulate the best care for those patients. Each month consultant from Chelsea and Westminster Hospital will join to aid the GPs/clinicians.

Kaleidoscope – End of life care Children

The Kaleidoscope service is a three pronged children’s palliative care service which helps support children and families to promote the best quality of life as possible. Professionals involved in the care of the child being referred. Specific medical evidence is needed but can be obtained from associated medical professionals.

They offer a drop-in service, providing cancer information and emotional support to anyone affected by cancer. The Macmillan Centre is open Mon–Fri, 9am–4pm in the Lower Ground Floor Outpatients Atrium (between lift banks C and D) at Chelsea and Westminster Hospital.

Marie Curie

Marie Curie provides support to everyone affected a terminal diagnosis to achieve the best quality of life, and support you to keep your independence and dignity for as long as possible.

Pembridge Hospice and Palliative Care

Local

Provide holistic palliative care for people who have been diagnosed with a life-limiting illness. Patients are referred to Pembridge Hospice by health or social care professionals. We accept referrals for patients with GPs based in the following CCG areas: Brent, Central London, Hammersmith and Fulham & West London.

Hay Fever

There has been a change in guidelines across the country. The NHS is asking patients to buy hay fever medication directly from the pharmacy.
